3rd Annual Servant Leadership Awards Banquet

This is an important reminder for all parishes to promptly submit the names and information of their award recipients for the 3rd Annual Servant Leadership Awards Banquet.
Please remember to upload your honorees’:
- Pictures & Biographies to the portal (due April 25th)
- Commemorative Album Form (due May 15th),
- Seating Arrangement Form (due May 15th).
Select two individuals who exemplify the spirit of servant leadership to be honored at this year’s banquet—preferably one adult and one young adult (ages 18-35). His Eminence will personally present each honoree with an award in recognition of their dedication and selfless service to the Church.
Please work on selecting your parish honorees as soon as possible and complete the online requests listed below:
- Upload a photo and a 100-word biography of your parish honorees using the provided portal. Click here to upload the Honorees' Bio.
- Fill out the Banquet Sponsorship Form to honor your parish honorees with a special message in the Commemorative Program.
- Click here to download the Sponsorship and Special Message form.
- Submit a Table Reservation Form for your parishioners to attend the event. Click here to download your table reservation form.
This year, we would love to have all past honorees join us and bring their awards for a group photo to mark the occasion. We kindly ask that honorees purchase tickets as usual, and we truly hope you will be part of this special moment.
